We operate across Worcester City and Wychavon.

We aim to provide services in a way that meets our clients’ needs.

We are funded through the Worcester City and Wychavon District Council’s and Supporting People, as well as charitable sources.

WHAT DOES THE SERVICE COST?

  • Advice from us is free.
  • Grant funding is means-tested (based on your financial circumstances), and you may have to pay towards the cost of the work. If this is necessary, you have to pay the contribution before the work is carried out and the money will be held in trust until the work is successfully completed.
  • The fee for grant-aided work will be a percentage of the cost of the work. The fee is normally met through the grant and should cost you nothing.

HOSPITAL DISCHARGE SCHEME

Cold or damp homes can cause accidents and illnesses. Sometimes, bathrooms, toilets and other areas of a person’s home become unusable or unsuitable and can put a person in danger. This can lead to health problems, depression, and anxiety, and this may result in them having to go into hospital. We run a Hospital Discharge Scheme, which works directly with patients both before and after they leave hospital.

WHO IS THE SCHEME AIMED AT?

  • Hospital patients who cannot leave hospital because of unsuitable or unsafe housing conditions.
  • People who are leaving hospital and having to cope with reduced mobility or a new physical disability.
  • Making a person’s home a safer place to live by carrying out repairs and adaptations that will help them falling and possibly having to go into hospital.

HOW DO WE RUN THE SCHEME?

For a number of older and disabled people returning home from hospital, their increased frailty, reduced mobility and loss of confidence can be made worse by problems in their home.

  • Somebody who can no longer climb the stairs or bath himself or herself properly may have to stay in hospital unnecessarily. Or, they can be discharged from the hospital to a home where their needs are not fully met, which puts them at risk again. Care & Repair’s Hospital Discharge Scheme can provide a speedy repair or adaptation service in the patients’ home.
  • We will advise patients on the various options that are available to them to allow them to return home.
  • We can identify areas of necessary work within a person’s property that would make their home a safer place to live.
  • We will help fill in application forms and paperwork to arrange funding for the repair or adaptation.
  • We will liaise with the Occupational Therapist in respect of the type of repair or adaptation.
